1. Contents of the policy that can be changed:
(1) Changes to the subjects and subject information of insurance contracts: changes to basic customer information, supplementary notifications, changes in occupation, changes in beneficiaries, changes in policyholders, etc.
(2) Changes to insurance contract contents: changes in payment methods, changes in payment frequencies, reduction of coverage, addition of additional insurance, reduction in payment amount, automatic advance payment, self-advance settlement, policy loans, policy repayments, changes in dividend claim options, changes in self-advance options, etc.
(3) Changes in the validity of insurance contracts and contract termination: policy reinstatement, policy surrender, etc.
(4) Insurance contract survival benefits: Annuity, maturity benefit, and survival benefit collection;
(5) Other maintenance services: policy loss reporting and reissuance, partial withdrawal from universal life insurance accounts, etc.
2. Channels for handling policy changes
(1) Entrust an agent to submit an application to our company (the application date shall be the time when our company receives your written application for change);
(2) Submit an application through the company's official website (limited to individual preservation items, such as changes to the customer's contact address, postal code, email address, etc.);
(3) Applications can be submitted by letter or by calling our customer service hotline at 400-889-6333 (limited to individual preservation items, such as changes to the customer's contact address, postal code, email address, etc.; the application date is based on the time when our company receives the change application).
(4) Come directly to our company to apply for processing.
3. Pay the renewal insurance premium
If you choose to pay the insurance premium in installments, after paying the first installment of the insurance premium, you should pay the renewal insurance premium to us on the due date of each renewal as agreed.
A grace period of 60 days commences on the day following the due date of each renewal premium. If you have not paid the premium by the end of the grace period, the insurance contract will be suspended or terminated from 0:00 on the day following the end of the grace period (except for "policies with automatic advance payment").
4. Reinstating the insurance policy: For insurance contracts with reinstating clauses, you can apply for reinstating the insurance policy within two years after the contract's validity is suspended. After our company's review and approval, and upon your payment of the outstanding insurance premiums and interest, as well as the policy loan and interest (if any), the validity of the insurance contract will be restored. If no agreement on reinstatement is reached within two years after the suspension of the policy's validity, the insurance contract will automatically terminate.
5. Apply for cancellation during the cooling-off period: For insurance contracts with a cooling-off period stipulated in the terms, you can apply for cancellation within 10 days from the day after you receive the insurance contract and sign for it in person. After receiving your application and the insurance contract you have returned, our company will fully refund the insurance premium you have paid without interest. This contract will be terminated upon our receipt of your application. We will not be liable for any insurance incidents that occur before the contract is terminated. (For details, please refer to the specific provisions of the "cooling-off Period" in the contract terms.
6. Processing policy cancellation:
During the validity period of the insurance contract, you may apply for policy cancellation. After receiving your application and the insurance contract, our company will refund the cash value of the policy at the time of contract termination, except as otherwise agreed in the contract.
7. Application for maturity insurance benefits, survival insurance benefits and annuities:
For insurance contracts where the terms stipulate maturity benefits, survival benefits or annuities, the insured may apply to Qianhai Life Insurance for the maturity benefits, survival benefits or annuities either by themselves or by entrusting others in writing.
The basic documents required for the application include
(1) The original insurance contract;
(2) Application Form;
(3) A copy of the beneficiary's (guardian's) bank account;
(4) Identity proof and survival proof of the insured (if the insured is a minor, the original ID card of the guardian and the proof of guardianship relationship must be provided simultaneously);
(5) If the matter is handled by an agent, an authorization letter and the identity proof of the agent must also be provided.
